Rates depend on the exact model and capacity, rental period, whether an operator and fuel are included, and transport to your site. Monthly hire usually works out cheaper per day than daily hire. Send one request and you get quotes from several suppliers to compare.

What is the minimum rental period?

Most suppliers rent by the day, week or month. Some heavy equipment has a minimum period because of transport costs. Mention your expected duration in the request so suppliers quote the right rate.

Is an operator included?

It depends on the equipment and the supplier. Many machines are offered with an operator; say in your request whether you need one, plus shift hours, so quotes are comparable.
